Most plans cover lab tests only when a licensed physician requests them. If you don't have a referral, our clinicians can assess whether the test is medically indicated, or you can proceed as self-pay with transparent pricing shown upfront.

Routine panels usually don't need one. Some specialized or high-cost tests do: when yours does, we submit the request to your insurer on your behalf and only proceed once it's approved, so nothing lands on you unexpectedly.

For in-network plans, only your co-pay or deductible: the exact amount is confirmed with you during eligibility verification, before any sample is collected. There are no additional lab fees on top.

We tell you before testing, never after. You'll see the exact self-pay price and can decide to proceed, swap to a covered alternative where one exists, or skip it: no surprise invoices, ever.

The lab tests themselves are billed to your insurer as usual. Some plans also cover the home-collection service; where they don't, the visit fee is shown to you at booking so you know the full cost upfront.

Yes. If you paid out of pocket, we prepare a reimbursement-ready pack (itemized invoice with standard billing codes, the physician's request, and attested reports) and our desk can help you fill in your insurer's claim form.

Bring it to our insurance desk. Rejections are often a coding or documentation issue we can correct and resubmit. We'll review the insurer's reason, fix what's on our side, and tell you plainly if the plan genuinely excludes the test.
