About This Test

What the Absolute Eosinophil Count test tells you

Eosinophils are white blood cells built for a particular kind of defence. Their granules contain proteins that are toxic to multicellular parasites, and the same machinery is deployed (far less usefully) in allergic inflammation. The absolute eosinophil count reports how many of these cells are circulating, as a number rather than as a share of the white cell count, and it is normally reported as one line of a full blood count differential rather than ordered on its own.

Eosinophils spend most of their life in tissue rather than in blood, particularly in the lining of the gut, airways and skin. A blood count is therefore a sample of what happens to be passing through, and it can understate disease that is confined to tissue. Their numbers also follow a daily rhythm driven by cortisol, sitting lower in the middle of the morning and higher overnight, and they drop sharply during acute bacterial infection and while corticosteroids are being taken.

A raised count is the finding that carries weight, and three explanations account for most of them. Allergic conditions (asthma, eczema, allergic rhinitis, food allergy) are the commonest. Drug hypersensitivity reactions are next, and can raise the count markedly within weeks of starting a new medicine. Parasitic infection is the third, and it is a genuinely relevant consideration in this region and for anyone returning from an area where soil-transmitted worms are common; it is a routine part of the differential, not a reflection on anyone. A markedly raised and persistent count also raises the possibility of eosinophilic organ disease, vasculitis or a blood disorder, and is investigated more thoroughly alongside the rest of the blood count and the clinical history.

Eosinophil numbers follow a daily rhythm, so if the count is being tracked over time try to give each sample at a similar time of day, ideally in the morning. Corticosteroids in any form (tablets, injections, strong inhalers or extensive topical use) suppress the count, so tell the team what you are taking and when you last took it rather than stopping anything on your own.

Mention recent travel, any new medication started in the preceding weeks, and any current infection, as all of these change how the result is read. The sample is collected into an EDTA tube.

Understanding Your Results

A result in the Normal band, reported in cells/µL, is the usual finding, and a very low or unmeasurable count is unremarkable in itself. Because eosinophils spend most of their life in tissue, an ordinary count can sit alongside active eosinophilic inflammation in the gut, airways or skin. A result in the High band most often reflects allergic disease such as asthma, eczema, allergic rhinitis or food allergy, a drug hypersensitivity reaction, or parasitic infection; a markedly raised or persistent count prompts a wider look at organ involvement and the rest of the blood count. Timing and treatment matter. The count follows a daily rhythm, and corticosteroids, acute bacterial infection and severe physical stress all push it down, sometimes to nothing, which can hide a genuine rise. Your doctor interprets the number alongside your symptoms, your medicines and your history.

Reading a High or Low Result
  • What a higher result can point to

    • Allergic disease: asthma, atopic eczema, allergic rhinitis and food allergy.
    • Parasitic infection, particularly with tissue-invasive worms.
    • Drug hypersensitivity reactions, which can raise the count markedly.
    • Eosinophilic disorders of the gut, lungs or skin.
    • Systemic vasculitis and some connective tissue diseases.
    • Blood disorders, including certain leukaemias and hypereosinophilic syndromes.
  • What a lower result can point to

    • Corticosteroid treatment, whether by tablet, injection or high-dose inhaler.
    • Acute bacterial infection or severe physical stress, both of which push eosinophils out of the circulation.
    • Conditions producing excess natural cortisol, such as Cushing's syndrome.

These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.

Limitations & Safety

What this test cannot tell you

  • A low or unmeasurable eosinophil count is a common and unremarkable finding, and is not in itself a sign of disease.
  • Blood eosinophils can be unremarkable while eosinophilic inflammation is active in tissue, so an ordinary count does not exclude conditions such as eosinophilic oesophagitis.
  • Steroid treatment suppresses the count and can mask a genuine rise, which matters when the test is done during a course of treatment.
  • The count varies through the day and between samples, so a single value is less informative than a trend.

Risks of the blood draw

  • Mild bruising or brief soreness where the needle went in.
  • Occasional lightheadedness, which settles after a few minutes of rest.
  • Bleeding or infection at the site, which is rare with sterile single-use equipment.
