What the Erythrocyte Sedimentation Rate (ESR) test tells you

The erythrocyte sedimentation rate is one of the oldest laboratory tests still in routine use, and its principle is simple. A sample of blood is placed in a narrow vertical column and left undisturbed, and the laboratory records how far the red cells settle away from the plasma over a fixed interval. When inflammation is present, the liver releases proteins into the blood, fibrinogen chief among them, and these proteins reduce the natural repulsion between red cells so that the cells stack together. Stacked cells are heavier than single ones and sink faster, so the distance they fall becomes an indirect measure of how much inflammatory protein is circulating.

The test's great strength and its great weakness are the same thing: it is entirely non-specific. A raised rate says that an inflammatory process is likely somewhere in the body, but it says nothing whatever about where that process is, what is causing it, or how serious it is. Infection, autoimmune disease, tissue injury, some cancers and several conditions involving no inflammation at all can produce the same picture. It is therefore never used alone to make a decision; it is used to support a suspicion, to help decide whether further investigation is warranted, or to follow something already diagnosed.

It also moves slowly. Unlike markers that respond within hours, the rate climbs over days and falls back over weeks, which makes it poorly suited to tracking a rapidly changing illness but well suited to monitoring a long-running one such as rheumatoid arthritis or polymyalgia rheumatica. That sluggishness is why doctors often order it alongside C-reactive protein, which reacts far more quickly, so that the two together give a sense of both the immediate and the sustained picture. Your doctor will read the result against your age, sex and general condition rather than in isolation.

Timing matters more than diet for this test: the sample must reach the laboratory promptly and be analysed within a limited window, because blood that stands too long or is kept warm settles differently and gives an unreliable rate. Tell the team if you are pregnant, currently menstruating, known to be anaemic, or taking anti-inflammatory or steroid medicines, since each of these shifts the result independently of the condition being investigated.

If the test is being repeated to follow progress, using the same laboratory each time makes the comparison more meaningful.

Understanding Your Results

A result in the Normal band, 0 to 20 mm/hr, means no significant inflammation was signalled on the day of the test. It does not exclude serious disease, because the rate climbs over days rather than hours. A result in the High band, 21 to 100 mm/hr, is a non-specific sign that an inflammatory process is likely somewhere: infection, rheumatoid disease or lupus, temporal arteritis, some cancers, chronic kidney disease or long-standing tissue injury. Several things move the rate with no inflammation involved. Age, female sex, pregnancy, anaemia and obesity raise it, while a high red cell count, abnormally shaped red cells and very high white cell counts slow the settling and lower it. Because it falls back over weeks, it suits a long-running condition better than a fast-moving one, and is often paired with CRP. Your doctor reads it against your age, sex, symptoms and history.

Reading a High or Low Result
  • What a higher result can point to

    • Infection, whether acute or long-standing such as tuberculosis or a deep abscess.
    • Autoimmune and rheumatic disease, including rheumatoid arthritis and lupus.
    • Temporal arteritis and polymyalgia rheumatica, in which the rate is often strikingly raised.
    • Anaemia and pregnancy, which speed settling for reasons that have nothing to do with inflammation.
    • Some cancers, particularly those involving the bone marrow and plasma cells.
    • Chronic kidney disease and long-standing tissue injury.
  • What a lower result can point to

    • A high red cell count, as in polycythaemia, which crowds the column and slows settling.
    • Abnormally shaped red cells such as sickle cells or spherocytes, which do not stack neatly.
    • Very high white cell counts, which interfere with the settling column.
    • Low fibrinogen, sometimes seen in advanced liver disease.

These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.

  • It is entirely non-specific: it suggests inflammation is present but says nothing about where it is, what is causing it, or how serious it is.
  • It responds slowly in both directions, rising over days and settling over weeks, so it lags behind the illness it reflects.
  • Age, female sex, pregnancy, anaemia and obesity all shift the rate independently of any disease, so the result is read against the person rather than as an absolute.
  • A low result carries little clinical meaning on its own and does not usually require explanation.
