What the Insulin, Random test tells you
A random insulin is a sample taken at any time of day, without fasting and without being tied to a particular meal or glucose drink. Its value therefore depends heavily on what you last ate and when, and it is not a suitable test for estimating diabetes risk or for screening for insulin resistance.
Its real usefulness lies in working out why blood sugar is low. A healthy pancreas switches insulin secretion almost off when sugar falls, so insulin that is still detectable at the moment of a low reading is a sign that something is driving secretion when it should have stopped. That may be an insulin-producing tumour of the pancreas, a medicine that stimulates the pancreas to release more, or insulin given by injection. Your doctor uses a C-peptide measurement alongside it to separate the body's own hormone from injected hormone, since the two cannot be told apart by an insulin assay alone.
Almost all of the meaning comes from the glucose measured at the same moment. The same insulin value can be entirely appropriate after a meal and clearly inappropriate during an episode of low blood sugar, which is why the pair is drawn together and why the ideal sample is taken while symptoms are actually present, before anything is eaten to relieve them. Insulin assays also differ between laboratories, so a value is best interpreted in the setting where it was measured. The result is a step in a wider investigation rather than an answer in itself.

Your doctor may order this test for any of the reasons below. Some are routine checks, others follow specific symptoms or an earlier result.
Episodes of low blood sugar
- Episodes of sweating, tremor, confusion or fainting that improve after eating.
- Documented low blood sugar with no obvious explanation.
Suspected insulinoma or medication effect
- Suspicion of an insulin-producing tumour of the pancreas.
- Concern that insulin or a pancreas-stimulating tablet has been taken by someone not prescribed it.
After weight-loss surgery
- Assessment of low blood sugar occurring after weight-loss surgery.
A random insulin, reported in µIU/mL, is read against the glucose measured in the same sample and never on its own. A result in the Normal band only reassures if the glucose it accompanies is also ordinary: the same value is entirely appropriate after a meal and clearly inappropriate during an episode of low blood sugar, because a healthy pancreas switches secretion almost off when sugar falls. A High result most often reflects a recent meal or ongoing insulin resistance. When blood sugar is low at the same moment, insulin that is still detectable points instead to something driving secretion when it should have stopped: an insulin-producing tumour, a tablet that stimulates the pancreas, or injected insulin, which the assay cannot separate from your own. C-peptide answers that last question. Your doctor reads the pair alongside your symptoms, your medicines and what you last ate.

What a higher result can point to
- An insulin-producing tumour of the pancreas, which secretes regardless of blood sugar.
- Insulin resistance, where high levels accompany normal or raised blood sugar rather than low.
- Injected insulin, or a medicine that stimulates the pancreas to release more.
- A recent meal, which raises the level in anyone and is the commonest reason for a high random result.
- Obesity, polycystic ovary syndrome or another state of chronic insulin resistance.
What a lower result can point to
- The normal response to low blood sugar, where a healthy pancreas correctly shuts secretion down.
- Type 1 diabetes or another autoimmune loss of the insulin-producing cells.
- Advanced type 2 diabetes with reduced beta-cell reserve.
- Pancreatic damage from chronic pancreatitis or surgery.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- A random level is only interpretable alongside the blood sugar measured at the same moment.
- Because it is not fasting, the result depends heavily on when and what you last ate.
- It is not a screening test for diabetes or insulin resistance; fasting or timed samples are used for that.
- Assays differ between laboratories and cannot distinguish injected insulin from your own, which is what C-peptide is for.
