What the Malarial Antigen, Rapid Detection Test test tells you

Malaria is caused by Plasmodium parasites carried by night-biting Anopheles mosquitoes. It does not spread within the UAE, but imported cases are seen regularly in residents and travellers returning from South Asia, sub-Saharan Africa and other regions where it is common. This rapid test looks for parasite proteins circulating in a small blood sample.

It works by capturing malaria-specific antigens on a test strip: the histidine-rich protein released by Plasmodium falciparum, and enzymes such as parasite lactate dehydrogenase and aldolase produced by the other human malaria species. Because it detects proteins rather than requiring a trained eye at a microscope, it delivers an answer quickly and at any hour. That speed is its purpose, and it matters in an illness where delay carries real risk.

Speed is not the same as completeness, which is where the companion test comes in. The rapid antigen test tells your doctor whether malaria antigen is present and often which broad group of species is involved. Microscopic examination of blood films goes further: it identifies exactly which species is present and how heavily the blood is infected, and both of those change treatment. Falciparum antigen can also stay detectable for weeks after successful treatment, so the rapid test cannot be used to confirm cure. The two are usually ordered together.

A single negative result does not exclude malaria when suspicion is high, and repeat testing is standard practice. Most importantly, fever after travel to an area where malaria is common needs prompt medical assessment: being seen by a doctor, not booking a test and waiting at home for the report. Malaria can deteriorate rapidly, and early treatment is what prevents that.

The sample can be taken whenever you can attend, without waiting for a fever spike, since antigen tests do not require the blood to be drawn during a peak. Bring a clear account of where you travelled and when you returned, whether you took antimalarial prophylaxis and how reliably, and whether you have taken any antimalarial or antibiotic treatment recently, because partial treatment can lower parasite levels and affect detection.

Reading a High or Low Result
