What the Prostate Specific Antigen (PSA), Free test tells you
Prostate-specific antigen does not travel through the blood in a single state. Some of it is bound to circulating proteins that hold it inactive, and some moves about unattached. The unattached portion is what a free PSA test measures. On its own that quantity says very little; its value comes entirely from the comparison with the total, and it is that relationship your doctor actually reads.
The reason the comparison is worth making is that benign and malignant prostate tissue tend to shift the balance in opposite directions. An enlarged but non-cancerous prostate tends to leave a greater share of the enzyme unbound, while prostate cancer is associated with a smaller unbound share. The difference is a tendency, not a rule, and the two populations overlap, so this test moves the probability rather than settling the question. Expressed the other way round: a reassuring free proportion makes benign enlargement more likely without excluding cancer, and a low free proportion raises concern without establishing anything.
Its practical place is narrow and useful. When total PSA is modestly raised and it is unclear whether tissue sampling is justified, the free fraction helps tip a decision that would otherwise be finely balanced, sparing some men a biopsy they did not need, and encouraging others to proceed. A urologist weighs it alongside the digital rectal examination, the size of the gland, your age, family history and increasingly MRI. It is one input into a judgement, and prostate cancer is still confirmed only on tissue.

Your report shows a single band, Normal, 0 to 2 ng/mL, but free PSA is not read on its own. What matters is the unbound measurement expressed as a share of the total PSA from the same sample: a larger unbound share points towards benign enlargement and is reassuring, while a smaller share raises the level of concern. Neither pattern settles the question, because only tissue sampling can answer it. Several everyday things move the figure. Recent ejaculation, hard cycling, a rectal examination or catheterisation shortly before the sample can lift it, as can prostatitis or a urinary tract infection. Medicines that shrink the prostate, such as finasteride and dutasteride, alter both measurements and must be declared. The unbound form is also less stable in the tube, so delays in handling distort the comparison. Your doctor reads it with your total PSA, your examination and your symptoms.

What a higher result can point to
- Benign prostatic hyperplasia, which typically leaves a larger share of the enzyme unbound.
- Prostatitis and other prostate inflammation, which lifts both the free and the total measurement.
- A urinary tract infection involving the prostate.
- Recent ejaculation, cycling, catheterisation or a rectal examination shortly before the sample.
- A simply large gland, since more prostate tissue releases more of the enzyme overall.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- Free PSA is only interpretable next to a total PSA from the same sample; the figure means little in isolation.
- The relationship between the free and total measurements shifts the probability of cancer but neither establishes nor excludes it: only tissue sampling can do that, and a raised PSA of any pattern is not a diagnosis.
- Medicines that shrink the prostate, such as finasteride and dutasteride, alter both measurements and must be declared.
- The free form is less stable in the tube than the total, so delays or poor handling between collection and analysis can distort the comparison.
