About This Test

What the Absolute Lymphocyte Count test tells you

Lymphocytes are the cells that give the immune system its memory. The population includes T cells, which direct and carry out cell-mediated defence, B cells, which mature into the plasma cells that make antibodies, and natural killer cells, which act early and without prior exposure. The absolute lymphocyte count reports how many of these cells are circulating, as a number rather than as a percentage, and it is normally reported as part of a full blood count differential and read as a pattern with the other cell lines.

The number moves for a wide range of reasons, most of them temporary. Viral infections typically raise it, sometimes markedly and with characteristic changes in the appearance of the cells on a blood film. Adrenaline, strenuous exercise and acute stress can lift it briefly by shifting cells from tissue into the circulation. It falls with corticosteroid treatment, chemotherapy, and during many acute illnesses. A shift in either direction shortly after an infection is extremely common and, on its own, usually means nothing at all, which is why an isolated abnormal count is normally just repeated once things have settled.

What matters is persistence and context. A sustained, unexplained rise in an adult (particularly alongside an enlarged spleen or lymph nodes) is investigated properly, since chronic lymphocytic leukaemia and lymphoma can present that way. A persistently low count raises questions about immune competence and prompts a look for causes ranging from medication to HIV to bone marrow disease. Neither conclusion is drawn from a single result; the count is always interpreted with the rest of the blood count and the clinical picture.

Strenuous exercise and acute emotional or physical stress temporarily raise the lymphocyte count, so avoid a heavy workout immediately before your appointment and rest for a few minutes before the sample is taken. Corticosteroids lower the count for a period after each dose: do not stop them, but tell the team what you take and when.

Mention any recent or current infection, any vaccination in the preceding weeks, and any chemotherapy or immunosuppressive treatment, since all of these move the count. Blood is collected into an EDTA tube and analysed promptly.

Understanding Your Results

A result in the Low band, reported in cells/µL, most often follows corticosteroid treatment, chemotherapy, a severe acute illness or major surgery; a count that stays low raises questions about immune competence and is looked into. The Normal band is the expected range and needs no action on its own. A result in the High band commonly reflects a viral infection and usually settles by itself, while a sustained, unexplained rise in an adult (particularly with an enlarged spleen or lymph nodes) is assessed further. Shifts in either direction shortly after an illness, strenuous exercise or acute stress are very common and often mean nothing, so an isolated abnormal value is normally repeated once things have settled. Reading a High or Low Result
  • What a higher result can point to

    • Viral infections, including glandular fever, cytomegalovirus and viral hepatitis.
    • Whooping cough, which characteristically raises lymphocytes.
    • Chronic lymphocytic leukaemia and other lymphoproliferative disorders.
    • Toxoplasmosis and some other non-viral infections.
    • A transient shift after acute stress, injury or strenuous exercise.
    • The period following removal of the spleen.
  • What a lower result can point to

    • Corticosteroid treatment, which redistributes and depletes circulating lymphocytes.
    • Chemotherapy, radiotherapy and immunosuppressive drugs.
    • HIV infection, and some other viral illnesses during their acute phase.
    • Severe acute illness, sepsis or major surgery.
    • Autoimmune disease, particularly systemic lupus erythematosus.
    • Bone marrow failure, or an inherited immune deficiency.

Limitations & Safety

What this test cannot tell you

  • Transient rises and falls after a recent illness are very common and usually mean nothing on their own, so an isolated abnormal value is normally repeated once the illness has settled.
  • The count says how many lymphocytes are present but nothing about which subsets they are; specialised testing is needed to separate T, B and natural killer cells.
  • An ordinary total count does not exclude a specific immune deficiency affecting one lymphocyte subset.
  • Recent corticosteroids or an acute illness can lower the count enough to hide an underlying abnormality.

  • Mild bruising or brief soreness where the needle went in.
  • Occasional lightheadedness, which settles after a few minutes of rest.
  • Bleeding or infection at the site, which is rare with sterile single-use equipment.
