What the Albumin Creatinine Ratio, Urine (Microalbumin) test tells you

This test looks for small quantities of albumin escaping into the urine: too little for a standard dipstick protein check to register, but enough to signal that the kidney's filters are under strain. The older name microalbuminuria refers to the size of the leak, not to a small kind of albumin; the protein is ordinary albumin, simply present in modest excess. Finding it is genuinely good news in one important sense: it identifies kidney damage at a stage when tightening control and adding the right medicines can slow it, and often reduce it.

What makes this version practical enough to repeat every year is that it needs only a single, untimed sample. Because a random sample can be dilute or concentrated depending on how much you have drunk, the albumin is expressed against the creatinine measured in the very same sample. Creatinine is produced and excreted at a fairly constant rate, so using it as the yardstick effectively cancels out hydration and yields a figure that can be compared fairly with last year's, without anyone having to collect urine across a whole day.

Its main role is annual screening in diabetes and in high blood pressure, where kidney damage is common, silent in its early stages, and modifiable if caught. One abnormal result, though, settles nothing. Albumin excretion rises transiently after vigorous exercise, during a fever or urinary infection, in heart failure and around menstruation, so an abnormal ratio is normally repeated on separate samples over the following months before it is taken to mean persistent kidney damage. Where precision matters more than convenience, a timed collection is used instead.

A first-morning sample is preferred where possible, because it is less affected by the day's posture and activity. Clean the genital area first and collect a midstream sample to avoid contamination.

Avoid strenuous exercise the day before, and postpone the test if you have a urinary infection, a fever, or are menstruating, since all of these raise urinary albumin without any kidney problem. Drink as you normally would rather than deliberately loading up with water.

Tell your doctor about blood pressure medicines, anti-inflammatory painkillers and any recent scan involving contrast dye, and bring last year's result so the two can be compared.

Understanding Your Results

A result in the Normal band, reported in mg/g, is what this annual screen hopes for and needs neither treatment nor investigation. The Microalbuminuria band means a small excess of ordinary albumin is escaping into the urine: kidney damage caught early, while tightening control and the right medicines can still slow or reduce it. A result in the High band reflects a larger leak and is followed up more actively. Measuring albumin against creatinine from the same sample largely cancels out hydration, though urine that is unusually dilute or concentrated can still skew a random sample. A urinary infection or fever, strenuous exercise the day before, heart failure, and contamination with menstrual blood or discharge all lift the ratio without kidney disease.
