What the Creatinine Clearance Test test tells you

Creatinine is a waste product generated continuously by muscle at a rate that is fairly steady for any given person. The kidneys remove it from the blood into the urine, and they do so largely by straightforward filtration. That combination (near-constant production and simple removal) is what allows creatinine to serve as a natural tracer for how well the kidneys are filtering.

The clearance test measures that directly rather than estimating it. Urine is collected over a full day and a blood sample is taken during the same period; comparing how much creatinine appeared in the urine with how concentrated it was in the blood gives the volume of blood effectively cleared of creatinine in a given time. That figure approximates the glomerular filtration rate, which is the single most useful summary of kidney function. Body size influences it, so results are usually adjusted for body surface area to allow fair comparison between people.

Most of the time this measurement is unnecessary, because a filtration rate estimated from a blood creatinine alone is quick, cheap and good enough. The measured version is reserved for situations where those equations are known to misfire: people at the extremes of muscle mass, amputees, those with advanced liver disease or unusual diets, pregnancy, and the assessment of a potential kidney donor. It is also used when a medicine with a narrow safety margin has to be dosed against real rather than estimated kidney function. Its accuracy, however, rests entirely on the collection. Missing even one passing of urine invalidates the result, and an incomplete collection is by far the most common reason the test has to be repeated.

No fasting is required, but avoid an unusually large meat intake and heavy exercise in the day before and during the collection, since both alter creatinine.

The collection is the test, so treat it carefully. On the start day, pass urine, discard it, and note the time.

From then on collect every single passing of urine over the next full day, finishing with one at the same clock time the following day: if even one is missed or accidentally flushed, tell the laboratory and start again on another day rather than submitting an incomplete collection, as this is the most common reason the test has to be repeated. Keep the container closed and refrigerated or cool throughout.

A blood sample is needed during or at the end of the collection period, so arrange that with the laboratory. Postpone if you have a urinary infection or are menstruating, and tell your doctor about anti-inflammatory painkillers, trimethoprim, cimetidine and any recent contrast scan, since these affect how creatinine is handled.

Understanding Your Results

A result in the Normal band, 90 to 140 mL/min, indicates healthy filtration. Mildly reduced, 60 to 89 mL/min, is common and often simply reflects age. Reduced, 0 to 59 mL/min, should be discussed with your doctor: chronic kidney disease, acute injury from dehydration or infection, obstruction to urine flow, heart failure and some medicines all lower it, and so does an incomplete collection, which is the commonest reason a clearance figure misleads. A higher than expected result has its own explanations: pregnancy, the early phase of diabetes, a high protein intake, greater than average muscle mass, or urine collected beyond the timed window. Reading a High or Low Result
  • What a higher result can point to

    • Pregnancy, in which filtration normally increases.
    • The early phase of diabetes, where the filtering units can work harder than usual before damage becomes established.
    • A high protein intake in the period before and during the collection.
    • Collecting more urine than the timed window actually covers.
    • Greater than average muscle mass, if the result has not been adjusted for body size.
  • What a lower result can point to

    • Chronic kidney disease from any cause, including diabetes and long-standing high blood pressure.
    • Acute kidney injury from dehydration, severe infection or a fall in blood pressure.
    • Obstruction to urine flow, for example from prostate enlargement or stones.
    • Medicines that reduce blood flow through the kidney, including some anti-inflammatory painkillers.
    • Heart failure or advanced liver disease reducing effective blood flow to the kidneys.
    • An incomplete urine collection, which falsely lowers the calculated clearance.

These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.

Limitations & Safety

What this test cannot tell you

  • The result is only as reliable as the timed collection; missed urine is the commonest reason a clearance figure misleads, and one missed passing invalidates the whole test.
  • A small amount of creatinine is secreted by the kidney tubules as well as filtered, so clearance slightly overstates true filtration, and does so more as kidney function declines.
  • Muscle mass, diet and several medicines affect creatinine independently of how the kidneys are working.
  • For most routine purposes an estimated filtration rate from a blood sample is sufficient and far easier to obtain reliably.

  • Mild bruising or brief soreness where the needle went in.
  • Occasional lightheadedness, which settles after a few minutes of rest.
  • Bleeding or infection at the site, which is rare with sterile single-use equipment.
