What the Thyroid Profile, Total test tells you
This panel reports three results from a single sample: thyroid stimulating hormone (TSH), total thyroxine (total T4) and total tri-iodothyronine (total T3). TSH is the pituitary's instruction to the thyroid, while the two hormone measurements describe what the gland has produced in response. The difference from the free profile lies entirely in what the hormone results include: a total measurement counts every molecule in the blood, both the large majority travelling attached to carrier proteins and the small unbound fraction that can actually enter cells and act.
That distinction is the main thing to understand about this panel. The amount of carrier protein in your blood is not fixed. Pregnancy and oestrogen-containing medicines raise it, while liver disease, protein loss through the kidneys, severe illness and certain inherited variations lower it. When carrier protein rises, the total hormone level rises with it even though thyroid function has not changed at all, and when it falls the reverse happens. A total result read without that context can suggest a thyroid problem where none exists, which is exactly why free hormone measurement is usually the preferred way to answer the question.
For this reason your doctor reads the hormone values against the TSH rather than alone, since TSH is not affected by carrier proteins and usually reveals whether the gland is genuinely over- or underactive. Total measurements remain useful in specific situations, including where interference is suspected in a free hormone assay, and total T3 in particular is helpful when an overactive thyroid is suspected but thyroxine looks unremarkable. Where the picture is unclear, free hormone testing is generally how it is settled.

What a higher result can point to
- Graves' disease or another cause of genuine thyroid overactivity.
- A toxic nodule or multinodular goitre making hormone independently of TSH.
- Thyroiditis releasing stored hormone from an inflamed gland.
- Pregnancy or oestrogen-containing medicines raising the carrier protein without altering thyroid function.
- Excessive or over-replaced thyroid hormone medication.
- An inherited excess of binding protein, which lifts the totals while the free hormones and TSH stay appropriate.
What a lower result can point to
- Hashimoto's thyroiditis or other autoimmune damage to the gland.
- Thyroid surgery, radioactive iodine or radiotherapy to the neck.
- Insufficient or irregularly taken thyroid hormone replacement.
- Loss of binding protein through kidney disease, liver disease or severe malnutrition, which lowers the totals without any underactivity.
- Severe illness, which reduces total T3 in particular.
- Pituitary disease, where low hormones are accompanied by a low or inappropriately normal TSH.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- Totals move with carrier protein levels, so pregnancy, oestrogen therapy, and liver or kidney disease can mimic thyroid dysfunction.
- Free hormone testing is generally preferred when the clinical question is whether the thyroid itself is working properly.
- High-dose biotin supplements and severe illness can both distort the results.
- Methods differ between laboratories, so serial results are best compared within the same laboratory.
