What the Amylase test tells you
Amylase is a digestive enzyme whose job is to break starch into smaller sugars. It is made in two places: the pancreas, which releases it into the small intestine, and the salivary glands, which start the process while you chew. Only a small amount normally circulates in the blood, so measuring it says something about whether either of those glands is inflamed or whether the enzyme is being cleared from the body less efficiently than usual.
The test earns its place in urgent care. When the pancreas becomes acutely inflamed, enzyme escapes from the damaged tissue into the bloodstream and the blood level climbs quickly, often within hours of the pain starting. That makes amylase a useful early test in someone with sudden, severe upper abdominal pain that bores through to the back, particularly when it comes with nausea and vomiting. It rises early but also falls back fairly quickly, so a sample taken several days into an illness can look deceptively unremarkable.
Amylase is not exclusive to the pancreas, which is why it is rarely interpreted alone. Salivary gland inflammation, a blocked salivary duct, mumps, a perforated or obstructed bowel, an ectopic pregnancy and reduced kidney clearance can all raise it. There is also a harmless quirk called macroamylasaemia, in which amylase binds to a large protein and cannot be filtered out by the kidneys, producing a persistently high blood level in a person who feels entirely well. For these reasons lipase is often measured at the same time, since it is more specific to the pancreas and stays elevated longer.
This test is ordered when you present with severe abdominal pain, nausea, or vomiting to rule out or confirm acute pancreatitis. It may also be used to monitor chronic pancreatic conditions.

Reasons your doctor may request the Amylase test
Your doctor may order this test for any of the reasons below. Some are routine checks, others follow specific symptoms or an earlier result.
Sudden abdominal pain
- Sudden severe pain in the upper abdomen, especially if it radiates through to the back.
- Persistent nausea and vomiting with abdominal tenderness.
Salivary glands & unexplained results
- Painful swelling of the salivary glands, or suspected mumps.
- An unexplained raised amylase found incidentally on an earlier test.
Known pancreatic disease & procedures
- Known chronic pancreatic disease being monitored over time.
- Assessment before or after procedures involving the pancreatic or bile ducts.
A result in the Low band, reported in U/L, is usually not a concern; it matters mainly where enzyme-producing tissue has been lost: chronic pancreatitis, cystic fibrosis, or pancreatic surgery. The Normal band is the expected range. A result in the High band most often reflects acute inflammation of the pancreas, but amylase also comes from the salivary glands, so mumps, a duct stone or gland inflammation raise it too, as can bowel obstruction, reduced kidney clearance and some medicines. A high level in someone who feels well may be macroamylasaemia, a harmless quirk in which amylase cannot be filtered out. Timing matters: amylase climbs within hours of the pain starting and falls back within days, so a late sample can look unremarkable, and its height does not show how severe the illness is. Lipase is measured alongside, and your doctor reads both with your symptoms and history.

What a higher result can point to
- Acute inflammation of the pancreas, which typically produces the most striking elevations.
- Salivary gland disorders, including mumps, a stone in the duct, or gland inflammation.
- Bowel obstruction, perforation, or reduced blood supply to the intestine.
- Reduced kidney function, since the kidneys clear amylase from the blood.
- Macroamylasaemia, where the enzyme binds a large protein and cannot be filtered out, despite normal health.
- Alcohol, certain medicines, and procedures that instrument the pancreatic duct.
What a lower result can point to
- Long-standing chronic pancreatitis in which much of the enzyme-producing tissue has been lost.
- Cystic fibrosis affecting the pancreas and its ability to secrete enzymes.
- Surgical removal of a substantial part of the pancreas.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- Amylase comes from the salivary glands as well as the pancreas, so a raised result is not specific to pancreatic disease.
- It rises and falls quickly, so a sample taken late in the illness can be unremarkable even when pancreatitis has occurred.
- The height of the result does not reflect how severe the illness is or how it will progress.
- Reduced kidney clearance and macroamylasaemia can both raise the level in the absence of any pancreatic problem.
