What the Triglycerides test tells you
Triglycerides are the body's main form of stored fat and its main way of moving fat energy around. Fat absorbed from a meal, together with fat the liver manufactures from surplus carbohydrate and from alcohol, is packaged into triglyceride-rich particles, carried in the blood, and either burned by muscle or deposited in fat tissue. Between meals those stores are released again, which is why the level in your blood swings considerably over the course of a single day.
A persistently raised level matters for two separate reasons. Over the long term it travels with insulin resistance, weight carried around the abdomen, fatty liver and type 2 diabetes, and it forms part of how cardiovascular risk is judged: partly because triglyceride-rich particles also deliver cholesterol into artery walls, and partly because a high triglyceride level usually keeps company with a low HDL. In the short term, a markedly raised level is a hazard in itself, because it can trigger acute inflammation of the pancreas.
Triglycerides respond to how you live more readily than most blood tests. Alcohol, refined carbohydrate, sugary drinks, weight change and physical activity all move the level within weeks, as do pregnancy, poorly controlled diabetes, an underactive thyroid and several common medications. That is why your doctor usually looks at a trend across repeat tests rather than reacting to one reading, and why a single unexpected result is often rechecked on a properly fasted sample.

Your doctor may order this test for any of the reasons below. Some are routine checks, others follow specific symptoms or an earlier result.
Routine cardiovascular screening

A result in the Normal band, 40 to 149 mg/dL, is a healthy level. Borderline high, 150 to 199 mg/dL, is a nudge rather than an alarm: alcohol, refined carbohydrate, sugary drinks, weight and activity all move it within weeks. A result in the High band, 200 to 500 mg/dL, keeps company with insulin resistance, poorly controlled diabetes, fat carried around the abdomen and fatty liver, and is also seen with an underactive thyroid, kidney disease, later pregnancy, and medicines such as corticosteroids and oestrogens. Triglycerides swing more across a single day than most blood tests, and a non-fasting sample can overstate the result, particularly after a fatty meal or alcohol. Illness, injury and recent surgery distort lipids for weeks. A single unexpected result is usually rechecked on a properly fasted sample, and your doctor reads the trend with your lipid profile and symptoms.

What a higher result can point to
- A diet high in refined carbohydrate or sugary drinks, and regular alcohol intake.
- Insulin resistance, poorly controlled diabetes and metabolic syndrome.
- Excess weight, particularly fat carried around the abdomen.
- An underactive thyroid, chronic kidney disease or nephrotic syndrome.
- Medications including corticosteroids, some diuretics, oestrogens and certain antiretrovirals.
- Inherited disorders of triglyceride handling, and pregnancy in the later months.
What a lower result can point to
- Prolonged fasting, undereating or significant malnutrition.
- Malabsorption from coeliac disease or other bowel conditions.
- An overactive thyroid, which accelerates the breakdown of fat.
- Rare inherited conditions in which very little of the carrier lipoprotein is produced.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- Levels vary considerably from day to day and after meals, so a single reading rarely settles anything.
- A non-fasting sample can overstate the result, particularly after a fatty meal or alcohol.
- Triglycerides describe fat transport, not the state of your arteries; imaging and clinical risk factors supply what the test cannot.
- Acute illness, injury and recent surgery distort lipid results for weeks.
