Glucose and HbA1c describe the result; insulin describes the effort behind it. Frequently asked questions
Anyone whose glucose and HbA1c keep returning normal while the symptoms persist: stubborn central weight, energy crashes after meals, or a PCOS or gestational diabetes history. Fasting insulin adds the one number those two markers cannot supply.
It shows the cost of keeping your sugar where it is. Two people can share an identical fasting glucose while one produces very little insulin to hold it there and the other produces a great deal: the second picture is what points your doctor towards insulin resistance.
The Diabetics Profile covers fasting glucose and HbA1c, and answers "is my sugar controlled?". This profile adds fasting insulin and answers "how hard is my body working to control it?": the earlier of the two questions, and the one that matters when sugar still looks normal.
It often is. Insulin resistance builds gradually, and rising insulin output can keep blood sugar in range long before HbA1c drifts up. That is precisely the window this profile is designed to look into, so a normal HbA1c does not make the insulin result redundant.
It covers the metabolic side of the picture, which matters because insulin resistance is common in PCOS and influences how it is managed. It does not include the reproductive hormones, so your doctor may order those separately alongside it.
The panel reports fasting glucose and fasting insulin, which are the two values a HOMA-IR calculation uses, and your doctor can read them together. If you want the index itself reported, HOMA-IR can be booked as a test in its own right.
Insulin climbs steeply within minutes of eating, and the reference ranges it is compared against are all built on fasting samples. A result drawn after breakfast cannot be interpreted against them, which is why the fast matters more here than for HbA1c.
Fasting insulin is difficult to interpret in that situation, because injected insulin forms part of what the assay measures. Speak to your doctor first: for people on insulin therapy the glucose and HbA1c pair is usually the more informative choice.
Eight to twelve hours, with plain water welcome throughout, since being well hydrated makes the draw easier. Avoid tea, coffee, juice, milk and chewing gum. Keep taking prescribed medicines, though anyone using insulin or a sulfonylurea should ask their doctor how to fast safely before booking.
