Inside the Liver Profile
A Liver Function Test in Dubai is usually ordered for one of two reasons: a symptom that will not settle, or a routine result that came back raised. This profile bundles ten tests in three groups: the enzymes alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT); the bilirubin fractions, total, direct and indirect; and the proteins the liver makes, albumin, globulin and total protein.
Each group answers a different question. ALT and AST leak out of injured liver cells, so they show damage as it happens, and ALT is the more liver-specific of the pair. ALP and GGT rise when bile flow is obstructed, and GGT is the more sensitive of those two to alcohol. Bilirubin is the pigment the liver clears: a raised indirect fraction leans towards red-cell breakdown or Gilbert's syndrome, a raised direct fraction towards the bile ducts. Albumin, globulin and total protein show what the liver is still manufacturing, which is how longer-standing damage tends to reveal itself. The report also carries the albumin/globulin ratio, calculated from results already in the panel at no extra cost.
One blood sample covers all of it. Fasting is recommended but not required: 8 to 12 hours gives the cleanest reading, since alkaline phosphatase can rise after a fatty meal. Water is fine, and take prescribed medicines as usual unless your doctor advises otherwise. Liver enzymes fluctuate, so one raised value is not a verdict. An abnormal panel is commonly repeated a few weeks later, once alcohol, a new medicine or a recent infection has been accounted for, and it is the pattern that matters: which enzymes moved, and how far. Changes that persist are then followed up with imaging or hepatitis testing.

Fasting helps, and note your medicines
Fasting is recommended but not required: 8 to 12 hours after your last meal gives the cleanest reading, and water is fine. Tell the phlebotomist about any medicine or supplement you take.

Ten tests: the enzymes ALT, AST, ALP and GGT, the three bilirubin fractions, and the liver-made proteins albumin, globulin and total protein. Together they describe liver cell injury, bile flow and the liver's manufacturing capacity.
Ongoing fatigue, nausea, poor appetite, discomfort under the right ribs, dark urine, pale stools or a yellow tinge to the eyes or skin. The panel is also used to monitor people on long-term medication that the liver has to process.
It helps assess it, but does not settle it. Fatty liver often raises ALT more than AST, yet enzymes can stay entirely normal while fat is present. Confirmation usually needs an ultrasound scan, which your doctor arranges after reading the panel.
Indirect bilirubin is the fraction the liver has not yet processed, and it rises with red-cell breakdown or Gilbert's syndrome. Direct bilirubin has been processed, and rises when bile cannot drain.
The liver manufactures albumin, so a falling level is one of the signs of long-standing liver disease: though it also drops in kidney disease, poor nutrition and inflammation. Globulin and total protein put that figure in context.
