What the Gamma Glutamyl Transferase (GGT) test tells you
Gamma glutamyl transferase is an enzyme anchored to the membranes of the cells lining the small bile ducts inside the liver, and it is also present in the pancreas, kidneys and intestine. Its job is to transfer glutamyl groups during the recycling of glutathione, one of the body's main antioxidant molecules. Only a small amount circulates in blood normally. The level rises when bile flow is impeded, when liver cells are irritated, or when the liver's enzyme systems are induced to work harder by alcohol or certain medicines.
In everyday practice GGT earns its place mainly as a discriminator. Alkaline phosphatase, another routinely measured enzyme, rises both in liver and bile duct disease and in bone conditions, growth and pregnancy. GGT is not present in bone, so measuring the two together resolves the ambiguity: a raised alkaline phosphatase accompanied by a raised GGT points towards the liver and bile ducts, while a raised alkaline phosphatase with an unremarkable GGT points away from them. This one comparison saves a great deal of unnecessary investigation.
GGT is extremely sensitive but not at all specific, and that combination is the source of most confusion about it. Alcohol raises it, but so do fatty liver disease, excess weight, insulin resistance, smoking, heart failure, several anticonvulsants and antibiotics, and many herbal preparations. An isolated raised GGT with an otherwise normal liver panel is a common finding and frequently does not indicate liver damage at all. It is best treated as a prompt to review alcohol intake, medicines and supplements, and metabolic health, sometimes with liver imaging, rather than as evidence of any particular cause.

What a higher result can point to
- Obstruction or slowing of bile flow from gallstones, strictures, tumours or drug-induced cholestasis.
- Regular or heavy alcohol intake, which induces the enzyme and can also injure liver cells.
- Fatty liver disease, excess weight and insulin resistance.
- Enzyme-inducing medicines such as some anticonvulsants and antibiotics, and many herbal and slimming preparations.
- Hepatitis of any cause, cirrhosis, and tumours or secondary deposits in the liver.
- Heart failure causing congestion of blood in the liver.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- GGT is very sensitive but highly non-specific: a raised value confirms that something is affecting the liver or bile ducts but never identifies which cause, and it is not proof of alcohol use.
- It falls only gradually after alcohol is stopped or a medicine is withdrawn, so a raised value can persist for weeks after the trigger has gone.
- A low or unremarkable GGT carries no established clinical significance, and the test is not used on its own to screen for liver disease.
