About This Test

What the Bilirubin, Total test tells you

Total bilirubin is the sum of the two forms of a single pigment produced when the body recycles old red blood cells. One form has already been processed by the liver and is on its way out in bile; the other is still circulating, bound to albumin, waiting its turn. Measuring them together gives one number that reflects the whole pathway from red cell breakdown, through liver processing, to excretion into the gut.

That breadth is the test's strength and its weakness. Because so many steps feed into it, a raised total bilirubin is a reliable prompt that something along the chain deserves attention, but it does not say which step. When the value is up, the doctor usually looks at the split between the conjugated and unconjugated fractions. A predominance of the unconjugated form points towards red-cell breakdown or a slow conjugating step; a predominance of the conjugated form points towards impaired bile flow inside or outside the liver.

Total bilirubin appears in almost every liver panel and in most general health checks, and it is also one of the components used in scoring systems that grade the severity of chronic liver disease. Visible yellowing of the skin and eyes only appears once the level has climbed well above its usual range, so a mildly raised figure in someone who feels perfectly well is common and frequently benign. Context (medicines, alcohol, recent illness, family history and the rest of the liver panel) carries most of the interpretive weight.

Fasting is not essential for bilirubin alone, but this test almost always travels with a liver panel or a full health check that does ask for an overnight fast, so follow the instruction attached to your booking.

Very long fasts can lift bilirubin on their own, so avoid stretching the fast far beyond what was asked. Keep drinking water, skip alcohol for a couple of days beforehand, and list every medicine, herbal product and supplement you take.

Let the team know about any recent infection, heavy exercise or blood transfusion.

Reading a High or Low Result

  • What a higher result can point to

    • Viral, alcohol-related, autoimmune or drug-induced inflammation of the liver.
    • Obstruction of the bile ducts by gallstones, a stricture or a tumour.
    • Accelerated breakdown of red blood cells from any cause.
    • Gilbert's syndrome, a benign inherited variation in bilirubin processing.
    • Advanced chronic liver disease with scarring of liver tissue.
    • Prolonged fasting or severe illness, which can raise the value temporarily.

These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.

Limitations & Safety

What this test cannot tell you

  • A low total bilirubin has no established clinical significance and is not treated as an abnormal finding.
  • The single number does not distinguish liver, bile duct and red-cell causes; fractionating it is what makes it useful.
  • The liver can be significantly damaged while total bilirubin is still unremarkable, so an ordinary value does not exclude liver disease.

Risks of the blood draw

  • Mild bruising or brief soreness where the needle went in.
  • Occasional lightheadedness, which settles after a few minutes of rest.
  • Bleeding or infection at the site, which is rare with sterile single-use equipment.
