What the Apolipoprotein B (Apo B) test tells you

Apolipoprotein B is the structural protein of every lipoprotein particle capable of depositing cholesterol in an artery wall: LDL, VLDL, IDL and Lp(a). Each of those particles carries a single Apo B molecule, and that fact is what makes the test useful: measuring Apo B counts the particles themselves rather than weighing the cargo inside them. LDL cholesterol, by contrast, measures how much cholesterol those particles happen to be carrying. Two people can have the same LDL cholesterol while one carries it in a few large, cholesterol-rich particles and the other in many small, cholesterol-poor ones, and it is the number of particles that determines how many can enter the artery wall.

This distinction matters most in exactly the people who are hardest to assess. In insulin resistance, type 2 diabetes and raised triglycerides, LDL particles tend to be small and cholesterol-depleted, so LDL cholesterol can look entirely acceptable while the particle count is high. Where the two measurements disagree, Apo B is increasingly regarded as the better guide to risk, and it also captures the contribution of triglyceride-rich remnant particles that an LDL cholesterol measurement largely misses.

For this reason Apo B is increasingly used to judge whether cholesterol treatment has done enough, particularly when LDL cholesterol has reached its intended level but concern about risk remains. It is not a substitute for the wider clinical assessment your doctor makes from blood pressure, glucose, smoking status, family history and, where appropriate, imaging. A raised result is a signal to look more carefully at overall risk rather than a statement that arterial disease is present.

Fasting is not required, since apolipoprotein measurements are relatively stable after meals.

If the sample is being taken with a full lipid profile that includes triglycerides, follow the fasting instruction for that panel instead. Sit quietly for a few minutes before the draw, and use similar timing and posture whenever the test is repeated, as this keeps successive results comparable.

Take lipid-lowering medication as prescribed (the purpose of monitoring is to see the treated level) and tell your doctor if the dose has changed recently. Postpone testing for several weeks after a heart attack, major surgery or significant infection.

Understanding Your Results

Apo B counts the particles able to deposit cholesterol in an artery wall, so a lower figure is the favourable direction. A result in the Optimal band, reported in mg/dL, carries the lowest cardiovascular risk. The Borderline band is where lifestyle change is usually advised. A result in the High band is a signal to look carefully at your overall risk, and points towards a diet high in saturated fat, excess weight around the abdomen, insulin resistance or diabetes, an inherited lipid disorder, an underactive thyroid, or kidney or cholestatic liver disease. Apo B is especially useful when triglycerides are raised, because LDL cholesterol can look acceptable while the particle count is high. Pregnancy, acute illness and recent surgery shift it temporarily, and effective lipid-lowering treatment brings it down. Your doctor reads it with the rest of your risk profile rather than alone.

Reading a High or Low Result
  • What a higher result can point to

    • A diet high in saturated fat, and excess weight carried around the abdomen.
    • Insulin resistance, type 2 diabetes and metabolic syndrome.
    • Familial hypercholesterolaemia and other inherited lipid disorders.
    • An underactive thyroid, nephrotic syndrome or chronic kidney disease.
    • Pregnancy, and medications such as corticosteroids and some antiretrovirals.
    • Cholestatic liver disease, in which lipoproteins accumulate.
  • What a lower result can point to

    • Effective treatment with statins, ezetimibe or PCSK9-directed therapy.
    • An overactive thyroid, which accelerates lipoprotein clearance.
    • Malnutrition, malabsorption or severe chronic illness.
    • Advanced liver disease, reducing lipoprotein production.
    • Rare inherited conditions in which very little Apo B is produced.

Limitations & Safety

What this test cannot tell you

  • Apo B counts particles but cannot show whether plaque has formed; imaging and clinical risk factors do that.
  • In people without metabolic disease it overlaps closely with LDL cholesterol and may add little.
  • Acute illness, recent heart attack, surgery and pregnancy shift the result temporarily.
  • It does not separate out Lp(a), which is measured on its own when that is the question.
