What the Apolipoproteins Profile (A1, B & Ratio) test tells you

This profile is a panel rather than a single analyte: it reports two apolipoproteins and the balance between them. Apo B is the structural protein of every particle that can deposit cholesterol in an artery wall (LDL, VLDL, IDL and Lp(a)) with one molecule per particle, so it works as a count of potentially harmful particles. Apo A1 is the defining protein of HDL, the particle that collects surplus cholesterol from tissues and returns it to the liver. The two therefore represent opposing arms of cholesterol traffic: delivery on one side, removal on the other.

The ratio expresses that balance as a single figure. A result tilted towards Apo B means comparatively more delivering capacity than removing capacity, and that direction is the one associated with higher cardiovascular risk; a result tilted the other way is the more favourable direction. Large international studies of heart attack risk have found this balance to track risk more closely than total or LDL cholesterol alone, particularly in people whose standard lipid panel looks unremarkable but whose clinical picture does not. Reporting the two components alongside the ratio matters, because the same balance can arise from a high Apo B, a low Apo A1, or both, and those situations call for different responses.

Sit quietly for a few minutes before the blood is taken, and try to keep the time of day and your posture consistent whenever the test is repeated so that comparisons are fair.
