What the Cholesterol, LDL (Direct) test tells you
Low-density lipoprotein is the vehicle that delivers cholesterol from the liver to the rest of the body. That delivery is necessary (cells need cholesterol for their membranes and for making hormones), but when there are more LDL particles in circulation than the tissues require, some of them lodge in the walls of arteries. Over years the trapped cholesterol draws in inflammatory cells and forms plaque, the process behind heart attacks, most strokes and reduced circulation in the legs. Of all the routine lipid measurements, LDL has the strongest and most consistent causal evidence behind it.
Most laboratories have historically calculated LDL from total cholesterol, HDL and triglycerides using an equation rather than measuring it. The equation works reasonably well in ordinary circumstances but becomes unreliable when triglycerides are high, when the sample was not taken fasting, or when LDL itself is very low: exactly the situations in which people are often already being treated. A direct assay measures the LDL fraction in the sample itself, which is why it is preferred in diabetes, in metabolic syndrome, and when monitoring someone on lipid-lowering therapy.
LDL is not read as a pass or a fail. What counts as an appropriate level for one person differs from another, because targets depend on overall cardiovascular risk: existing heart disease, diabetes, blood pressure, smoking, family history and age all shift where a doctor wants the value to sit. The same figure can be entirely reasonable in a young person with no other risk factors and clearly too high in someone who has already had a cardiac event. Discuss your result in that context rather than against a single universal figure.

Routine lipid screening

When calculated LDL is unreliable
- Diabetes, prediabetes or metabolic syndrome, where calculated LDL is less reliable.
- Known raised triglycerides, which distort the calculated estimate.
Treatment, family history & known disease
- Ongoing treatment with a statin or another lipid-lowering medicine.
- Family history of early heart disease or of inherited high cholesterol.
- Established cardiovascular disease requiring close follow-up.
A result in the Optimal band, 40 to 99 mg/dL, carries the lowest cardiovascular risk. Near optimal, 100 to 129 mg/dL, suits most people who have no other risk factors. Borderline high, 130 to 159 mg/dL, usually prompts a conversation about diet, weight and activity. High, 160 to 250 mg/dL, is worth discussing with your doctor, who may look for an inherited lipid disorder, an underactive thyroid, kidney disease or poorly controlled diabetes behind it. LDL is not read as a pass or a fail. - Obesity, physical inactivity and poorly controlled diabetes.
- Certain medicines, including some diuretics, steroids and immunosuppressants.
What a lower result can point to
- Treatment with statins or other lipid-lowering medicines working as intended.
- An overactive thyroid gland, which speeds up LDL clearance.
- Malnutrition, malabsorption or significant unintentional weight loss.
- Advanced liver disease, since the liver produces these particles.
- Severe acute illness, infection or the period following major surgery.
- Rare inherited conditions that keep LDL very low lifelong.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests: your doctor decides what it means for you.
What this test cannot tell you
- A single value does not describe risk on its own; it is interpreted with blood pressure, glucose, smoking status, age and family history.
- LDL measures the cholesterol carried, not the number of particles carrying it, so apolipoprotein B can add information in some people.
- Acute illness, infection, injury and the weeks after a heart attack lower lipid values temporarily and make that timing a poor moment to test.
