31 analytes from a single urine sample: 19 bacterial and yeast organisms found in urinary-tract infection (Escherichia coli, Klebsiella, Proteus mirabilis, Enterococcus, Staphylococcus and Candida albicans among them), and 12 antibiotic-resistance genes. Each one is reported individually.

A culture has to grow the organism before it can name it, which takes time and fails when the organism is fastidious or already suppressed by antibiotics. PCR detects the DNA directly, so it reports organisms a culture can miss. A culture, in return, measures how the organism responds to each drug.

A piece of bacterial DNA that lets bacteria survive a particular family of antibiotics. CTX-M covers many penicillins and cephalosporins (the ESBL pattern); mecA is the MRSA marker, vanA and vanB affect vancomycin, and the bla genes affect the carbapenems. Finding one flags which families may not work.

It narrows the field by naming the organism and flagging resistance families, but it is not a culture and sensitivity result: it cannot say which organism in the sample carries the gene, nor whether that gene is active. Your doctor chooses the drug from this, your history and your symptoms.

There are a few usual explanations: the original organism was never fully treated, a second organism was present and left untreated, the antibiotic chosen was one the organism resists, or the cause was never bacterial. Naming the organism and its resistance markers separates those possibilities instead of repeating the same empirical course.

Often. A sterile culture alongside real symptoms usually means the organism did not grow: it may be fastidious, present in small numbers, or suppressed by antibiotics you have already taken. PCR does not need growth, so it can put a name to that picture.

You can, because PCR still detects the DNA, but tell your doctor and our team since it changes how the result reads. Antibiotics reduce the organisms without necessarily removing their genetic material, so a detection may reflect an infection already being treated. Where symptoms allow, sampling before treatment starts is cleaner.

Candida albicans is included, which matters because a yeast does not respond to an antibiotic and turns up after repeated antibiotic courses, in diabetes and after catheterisation. Other Candida species are not on this panel: STD Master 28 covers a wider set of them.
