Urine Analysis

Urinalysis (Routine)

A routine urinalysis examines the colour, clarity, pH, and chemical composition of urine along with microscopic sediment. It screens for urinary tract infections, kidney disease, and diabetes.

About This Test

What the Urine Analysis test tells you

A routine urinalysis is not a single measurement but a structured examination of one urine sample in three stages. The sample is first inspected for colour and clarity. A reagent strip is then used to test it chemically for acidity, concentration, protein, glucose, ketones, blood, bilirubin, urobilinogen and two markers associated with infection. Finally the sample is spun down and the sediment examined under a microscope for red cells, white cells, bacteria, crystals, casts and cells shed from the lining of the urinary tract.

Because it is largely descriptive, the report is read as a pattern of findings rather than as a value that is higher or lower than it should be. White cells together with bacteria and positive infection markers point towards a urinary tract infection, while white cells alone suggest inflammation without one. Protein with red cells and cellular casts points to the kidney's filtering units rather than the bladder. Glucose with ketones raises the question of poorly controlled diabetes. Each finding is weak evidence on its own; read together they narrow the possibilities quickly and inexpensively, which is why urinalysis remains one of the most widely used first-line tests in medicine.

What it does not do is provide an answer. Casts and crystals appear in healthy people, a trace of blood may come from something as ordinary as menstruation or vigorous exercise, and a concentrated sample exaggerates almost everything on the strip. Findings that matter are confirmed by other tests: a suspected infection with a urine culture, protein with a quantified measurement, persistent blood with imaging or specialist referral. Your doctor reads the report against your symptoms and history and decides what, if anything, needs following up.

Why Get Tested?

General health screening, investigating urinary symptoms (pain, frequency, colour changes), monitoring kidney disease, and pre-surgical evaluation.

  • Wide, rapid screen

One sample gives information about infection, kidney function, glucose handling and bile pigments at the same time.

  • Investigates urinary symptoms

Burning, frequency, urgency, pain or a change in urine colour are assessed here first.

  • Detects silent findings

Protein, blood or glucose frequently appear in urine before any symptom is noticed.

  • Points towards further testing

It is designed to indicate which direction to look next rather than to settle a question by itself.

When This Test Is Ordered

Reasons your doctor may request the Urine Analysis test

Your doctor may order this test for any of the reasons below. Some are routine checks, others follow specific symptoms or an earlier result.

  • Urinary symptoms & appearance

    • You have burning, frequency, urgency or pain on passing urine.
    • Your urine has changed colour, become cloudy or persistently foamy.
    • Unexplained flank or back pain, fever or fatigue is being investigated.
  • Health checks, pregnancy & pre-operative assessment

    • A general health check, pregnancy review or pre-operative assessment is being carried out.
  • Diabetes, blood pressure & kidney stones

    • Diabetes or high blood pressure is being monitored for kidney involvement.
    • You have had kidney stones before and crystals or blood are being looked for.

A first-morning sample is best because it is the most concentrated and shows abnormalities most clearly, and it should reach the laboratory promptly, since urine left standing changes chemically and its cells break down. Wash first, then collect a clean midstream sample: pass the first part of the stream into the toilet and fill the container without touching the inside.

Postpone the test if you are menstruating unless your doctor advises otherwise, because blood contaminates the sample and changes what is seen. Avoid vigorous exercise beforehand, and mention vitamin supplements, urinary antiseptics and antibiotics, all of which can alter the strip readings.

Limitations & Safety

What this test cannot tell you

  • It is a descriptive screen rather than a diagnosis, and important findings need confirming with culture, quantification or imaging.
  • Results depend heavily on how the sample was collected, how concentrated it was, and how long it stood before testing.
  • Menstruation, recent vigorous exercise, vaginal discharge, and some medicines and vitamin supplements can all produce misleading findings.
  • The report has no meaningful high or low direction; it is a set of observations that are either present or absent.

Risks of the blood draw

  • Mild bruising or brief soreness where the needle went in.
  • Occasional lightheadedness, which settles after a few minutes of rest.
  • Bleeding or infection at the site, which is rare with sterile single-use equipment.
Key Points
  • 1Urinalysis combines visual, chemical and microscopic examination of a single sample.
  • 2The meaning lies in the pattern across findings, not in any one line of the report.
  • 3A clean midstream sample delivered promptly gives by far the most reliable result.
  • 4Menstruation can affect the findings, so testing is usually postponed until afterwards.
  • 5It is a broad first-line screen that points towards further testing rather than an answer in itself.

The first part of the stream washes cells and bacteria from the opening of the urinary tract into the container, producing findings that have nothing to do with the bladder or kidneys. Discarding it makes the sample far more representative.
